Columbia COVID19 Questionnaire

These files include the Columbia COVID-19 Questionnaire version 2.3 for electronic self-report data collection from patients with confirmed or suspected SARS-Cov2 virus infection. The questionnaire is currently being used by the COVID-19 Columbia University BioBank(CUB) and has been adopted for a variety of host genetic studies of COVID-19 complications nationally and internationally. We include translations into the following languages: English, Spanish, Italian, French, German, Polish, Japanese, Chinese and Korean.

RedCap Implementation

Columbia COVID19 Questionnaire along with the e-consent form was implemented on RedCap. The RedCap codebook and data dictionary for version 2.3 can be found below. Additional updates and newer versions will be posted on GitHub.

Acknowledgements

The Columbia COVID19 Questionnaire was assembled and harmonized by Krzysztof Kiryluk and Wendy Chung on behalf of the Precision Medicine Resource (PMR), with additional input from several departments and divisions at Columbia University, including Medicine (General Medicine, Pulmonary & Critical Care, Nephrology, Cardiology, Infectious Disease, Gastroenterology, Rheumatology), Neurology, Pediatrics, Obstetrics & Gynecology, and the Mailman School of Public Health. Please note that this questionnaire was designed for administration in the United States; the translations may require further tailoring based on the local health care setting.
